A4 portrait format typewritten in black on three pages of pre-printed white business stationery, holepunched (the lower section probably formerly a reply slip, now torn off). Stapled to an additional typewritten sheet listing the Practitioner Performance Bonuses from October 1980 and an explanation of the annual salary review.
